What is PACT?

A Patient Aligned Care Team is a group of health care professionals. They work together to give you the best possible care possible. This team also helps you plan how you want to participate in your health and wellness.

What is a provider?

A provider is a doctor or a nurse practitioner.

How can I contact my provider?

The best way to contact your provider is to call the Patient Response Center (PRC) at 317-988-1772 or 988- PRC. A message will be sent to your PACT team RN who will return your call. Every effort is made to return the call on the same day, occasionally for non-urgent needs vour call mav be returned the next business dav.

Can I just stop by and have a "walk-in" appointment?

Walking in to clinic can result in long wait times and there is no guarantee you will be seen by your provider. Please call PRC at 317-988-1772 or toll free 1-888-342-7602 and leave a brief message for your clinic regarding the medical problem. The clinic RN will return your call to further assess your medical need. Calling the clinic to request an appointment rather than walking in to the clinic will greatly reduce your wait time to be seen.

What is Secure Messaging?

Veterans now have the ability to send a secure message (e-mail) to their provider through My HealtheVet for non urgent/non life threatening medical concerns. Please contact your clinic staff for more information on My HealtheVet and secure messaging.

Why do I have a nurse or pharmacy appointment scheduled?

The nurses and pharmacists help manage chronic health problems. Providers in your clinic work closely with the nurses and pharmacists to assist you with your medical needs. This team effort can greatly
reduce long term side effects and problems as well as increase vour qualitv of life.
