Home Telehealth
(RN Care Coordinators)

Patients are taught how to self-manage their health conditions, develop goals to work on from home and upload data/response to questions daily.

Benefits for patients:

Mental Health:
     -  Depression
     -  PTSD
     -  Anxiety
     -  Substance Abuse

Video on Demand/VA Video Connect

 VA Video Connect is a new VA solution that enables Veterans to virtually meet-up with their VA healthcare providers, in a virtual medical room, using encrypted video to ensure session is secure and private.  It allows Veterans to see and talk to their health care team from anywhere, making VA healthcare more convenient and reducing travel times for Veterans, especially for those in very rural areas with limited access to VA healthcare facilities.

Options for use:
     -  In place of phone visit with advantage of actually seeing patient
     -  Same day visits if needed

Store and Forward:
     -  Images taken, stored and forwarded by providers in house or to another site;
          -  Teledermatology
          -  Teleretinal imaging

Telehealth Diabetes Program

Veterans with A1C greater than 7

Uses a home messaging and monitoring device to interact with telehealth nurse via the Veteran's home to self-manage diabetes.

TeleMOVE

Veterans with BMI greater than 25

Uses a home messaging and monitoring device to interact with a dietitian via the Veteran's home to self-manage weight loss.
