Actively Caring for Diabetes Control (ACDC)

Acupuncture-Complimentary Integrative Health (CIH)

Veterans with Chronic Musculoskeletal Pain and Headaches

The Provider orders a PMRS Chiropractor New Evaluation consult for acupuncture therapy when other therapies are inappropriate or have failed, or when acupuncture therapy is needed in conjunction with an integrated treatment plan. The chiropractor will clinically review the consult.

Diabetes 101

Group meeting to learn about diabetes, why blood sugar control is important, counting carbohydrates, what and how much to eat, and how to avoid diabetes complications.  

HIV Testing and Prevention

Lab work and monitoring of patients will be done by the infectious disease specialists. Patients are offered HIV prevention education, prophylaxis and discuss risk reductions strategies. Patients are followed for 3-months while they are on prophylaxis.

Individual Tobacco Sensation Session

Sessions for patients unable to make the group classes or who need more assistance. Nicotine replacement therapy may be prescribed by the patient's physician.

Indy West Hypertension Group

Insomnia Group

Kidney Disease Support Group for all Veterans with kidney disease

Each meeting has a short educational presentation the meeting is opened for group questions and discussion. Veterans may self-refer.

Kidney Failure Treatment Options Education

Tuesdays 10:00am-12:00pm and Thursdays 1:00pm-3:00pm

Education opportunity to learn about treatment options for kidney failure including dialysis, kidney   transplant and conservative care. Veterans may self-refer.

Nicotine Replacement Medications

Contact PACT RN, Provider, Health Behavior Psychologist

Patient's primary care team can provide brief counseling, nicotine replacement (patches/gum/lozenges), and/or other medications to help quit smoking.

OEF OIF OND Care Management and Readjustment Recovery Services

Monday-Friday 
8:00am-3:30pm

Returning Veterans are assisted with readjustment and recovery. Individual, family and group counseling services are available. E.g. couples' retreat, substance abuse group, women's support, PTSD/spirituality group, grief group, and more.

Physical Therapy Home Exercise Program (HEP)

Veterans with pain for new conditions

Provider places Physical Medicine Physical Therapy Outpatient consult

Home exercise therapy program for patients with a new condition.   Patients should note that exercises taught are for long-term use and to continue exercises until told to do otherwise.

Quit Tobacco Workshop

Wednesdays 1:00pm-3:00pm

Patients attend weekly group and may start and end the program based on their needs. Nicotine replacement/medicines may also be included as part of treatment.

Visual Impairment Services Team (VIST)

What is VIST?

VIST stands for Visual Impairment Services Team.  It is a program for Veterans who have vision loss.  By coordinating VA resources with community resources, Veterans and their families are helped to adjust to vision loss, blindness and severe visual impairment.

How do I know if a Veteran is eligible?

Any Veteran who is eligible for VA health care and who struggles with vision loss is eligible for VIST.

What Services are available to Veterans?

- Assessment of Veteran's individual rehab and training needs.
- Support and adjustment counseling.
- Referral to VA Blind Rehab Centers and low vision clinics.
